The Personal Outcomes Evaluation Tool (POET) is a short, anonymous questionnaire for parents, carers, children and young people and anyone who works with children and young people who receive additional SEN support, or have an Education Health and Care Plan (EHCP).

The survey is run by an independent charity called In Control. The results from the survey will be used to help shape and improve the service that Surrey County Council provides for its children, young people and families.

There are separate questionnaires for children and young people, as well as parents and carers to complete. Please could all schools encourage parents, carers, children and young people who receive additional SEN support, or have an EHCP to complete the survey.
